Law/Patents - Law: Contract(s)
English term or phrase: release letter
This is a letter in which someone (probably an actor/actress, or an extra)grants consent for filming, taking pictures, etc. . It also includes an assignment of copyright. It looks like a contract, with personal information, date and signature left blank.

My best option is 'vrijwaringsbrief' but I'm not sure.
