Diagnostics and treatment of distal tibia fractures

Резюме. Переломы дистального отдела голени составляют 10% переломов нижней конечности и встречаются у молодых активных пациентов (30–40 лет) в результате высокоэнергетичной травмы. Тяжесть повреждений обусловлена незначительной толщиной мягкотканных покровов (высокая доля открытых повреждений и раневых осложнений в послеоперационном периоде), вовлечением суставной поверхности и возникновением дефектов кости вследствие вдавления. Традиционные методы хирургического лечения сопровождались раневыми осложнениями в 11–25% случаев, нарушениями консолидации до 9%, высокой частотой внутри- и околосуставных деформаций (до 42%). Понимание роли мягкотканных покровов, применение этапной тактики лечения (временная внешняя фиксация с окончательным остеосинтезом только после нормализации состояния мягких тканей через 12–21 сут) и методов минимально-инвазивного остеосинтеза, появление анатомически предызогнутых имплантатов с блокированием винтов позволили снизить частоту тяжелых осложнений.

Summary. Distal tibia fractures comprise about 10% of all lower limb fractures and occur predominantly in young active people (30–40 y.o.) as a result of high-energy injury. The severity of injury depends on thin ***soft-tissue envelope*** (high percentage of open fractures and wound complications post-operatively), involvement of joint surface and bone defects due to impression. Traditional surgical treatment was accompanied by wound complications in 11–25% of cases, delayed and non-unions in 9%, intra- and periarticular malunions up to 42%. Understanding of the importance of soft-tissues, the introduction of staged treatment protocols (temporary external fixation and subsequent definitive fracture fixation only after soft-tissue recovery 12–21 days later), use of minimally-invasive fixation techniques and appearance of anatomically preshaped locked implants allowed to decrease the number of severe complications.
