French term or phrase: tuteur gardien légal
I'm translating a referral form for mental health care services in Ontario.
There is a section on the form concerning the "répondant", asking for his/her name and including checkboxes to indicate whether the respondent is a parent, tuteur or gardien légal. That's all the context.

I'm not sure what the difference between tuteur and gardien légal would be.

Assuming the context is Canada, I have found a few government websites where "tuteur légal" is translated as "legal guardian", like this one.

Termium has "guardian" for "tuteur" https://www.btb.termiumplus.gc.ca/tpv2alpha/alpha-eng.html?l... but with the following useful note:

"Depending on the provincial or territorial law, the guardian is responsible for making decisions about the person or the estate of the person or both the person and the estate."

So it depends on the province or territory in question, and we need this detail to investigate further.

As a one-size-fits-all solution, I would suggest the answer I posted.

  -> Yes, thanks for pointing that out Eliza, my full answer should read "holder of parental authority / legal guardian". I thought that the translation of the second term was obvious, as it is a direct calque from the English.
