Translation:

Defining an annual plan of activities enables more/greater interaction between partners, apart from promoting spaces to launch interventions acccording to a "support approach towards the most vulnerable""


En este caso: Asocio el cliente lo traduce como "partnering"; sin embargo, estoy haciendo una pequeña adaptación para que tenga un poco más de sentido en inglés.

Estoy más que consciente que el término "intervention" en inglés tiene una connotación un tanto negativa; sin embargo, lo estoy traduciendo literalmente puesto que es el término que utiliza el cliente en inglés.

Mas que traducir el término consultado, lo estoy interpretando en base al contexto.
