The transplanted kidney can be referred to as "transplant" or "graft". The point here is that injerto (graft) refers to the kidney (assessment of both the graft/transplant and native kidneys) rather than to the scan.
Or that is what I first thought, in the given context.
Re-reading, it could well be a technical description of the scan itself as Douglas suggests (that will show to be unnecessary to translate here), being conveyed that a renogram was performed in a patient either with or without a kidney transplant ("unspecified"). I had not seen this with renograms but this way of naming a procedure is rather common within radiological exams in Spanish -to be thus specified or unspecified. If this was the case, it would only be confusing to translate other than renogram. And if the patient does really have a kidney transplant, the first version (assessment of both kidneys, transplant [graft] and native) might be useful after all.
