Source text - Norwegian Aftenposten kunne 10. juni i år fortelle at et utvalg narkomane ble pålagt å trappe ned Subutexbehandling i rekordfart, på tross av legemiddelfirmaets advarsler. Noen av de narkomane døde. Stipendiaten som gjennomførte Subutexforskningen forklarte at hun stolte på veilederne sine, og hun antok derfor at eksperimentet var medisinsk og etisk forsvarlig. Forskningen var dessuten godkjent av Regional etisk komite for medisinsk forskning (REK).
All forskning som involverer medisinske opplysninger skal godkjennes av REK. REK har strenge regler for godkjenning, og det viktigste kriteriet er frivilligheten. Det skal ikke være noen tvil om at samtykke til deltakelsen er gitt på frivillig og informert grunnlag.
Hva med ambulansesjåførene og stipendiaten som deltok? Er de medskyldige eller ofre? På hver sin måte godtok de autoritetenes ordre og råd, sannsynligvis på tross av bedre viten. Trenet ambulansepersonell vil normalt vurdere situasjonen før han eller hun avgjør behandlingen. Burde de ikke visst bedre enn å godta tilfeldig loddtrekning som erstatning for sin egen kyndige vurdering av situasjonen og pasienten? Og hvorfor stolte stipendiaten på andre enn seg selv?
Å godta at egen kompetanse og vurderingsevne settes til side for å delta i "vitenskapelig forskning" minner om de berømte Milgram-eksperimentene som ble utført på 60-tallet.
Forskeren Stanley Milgram fikk helt vanlige mennesker til å gi falske forsøkspersoner (skuespillere) "dødelig" dose strøm i en pedagogisk, forskningsmessig hensikt. Skuespillerne hylte av "smerte" mens de egentlige forsøkspersonene økte strømdosen for å lære "dem å huske bedre".
Milgram beviste gjennom en serie slike eksperimenter at vanlige menneskers 25 lydighet overfor autoriteter er mye større enn hva vi selv tror. Helt vanlige mennesker kan være villige til å drepe i en gitt situasjon. De egentlige forsøkspersonene til Milgram visste ikke hva de var med på, og måtte resten av livet leve med sannheten om at de var villige til å påføre andre mennesker smerte og død, av ren lydighet. Selv om de deltok frivillig, ble integriteten deres krenket.
Milgrams forskning beviste at vi alle er i stand til å utføre grusomme handlinger hvis autoriteter påtar seg ansvaret. Dette har hatt betydning for blant annet forståelsen av krigsforbrytelser. Var det rett av Milgram å utføre forskningen sin? Ville du deltatt selv?
Det må stilles flere spørsmål. Spørsmål om etikk og forskning handler om retten til 35 selvbestemmelse, om ikke å få integriteten krenket, og om å bli beskyttet mot overgrep.
Det handler ikke bare om retten til ikke å delta, det handler også om retten til å delta. Det handler om rett og plikt til å si nei til autoriteter og forskere, og kanskje risikere sanksjoner, latterliggjøring eller oppsigelse.
I siste omgang handler dette også om hvordan samfunnet bruker ny kunnskap. Brukere av kunnskap på alle nivåer kan og bør stille spørsmål om forskningsetikk, og om hvordan vi bruker resultatene av eventuell uetisk forskning.
Translation - English On June 10th this year, Aftenposten reported that selected drug addicts were ordered to reduce their Subutex use over a very short time, despite warnings given by the pharmaceutical company. Some of the addicts died. The research fellow who conducted the Subutex study explained that she trusted her supervisors, and that she therefore assumed that the experiment was medically and ethically justifiable. Furthermore, the research was approved by the Regional Ethics Committee for Medical Research (REK).
All research involving medical information must be approved by the REK. The REK has strict rules for this approval, the most important criterion being free will. There should be no doubt that consent for participation is given on a voluntary and informed basis.
So, what about the ambulance drivers and research fellows that participated? Were they accomplices, or victims? They each accepted the authority's orders and advice, despite probably knowing better themselves. Trained emergency personnel would normally evaluate the situation before he or she determines the treatment. Shouldn’t they have known better than to accept random lots in place of their own expert assessment of the situation and the patient? And why did the research fellow trust anyone other than themselves?
To accept that one’s own skills and judgment can be set aside in order to participate in ‘scientific research’ is reminiscent of the famous Milgram experiments that were performed in the 1960s.
The researcher Stanley Milgram persuaded completely ordinary people to administer ‘lethal’ doses of electricity to false participants (actors) in the interests of education and research. The actors were screaming in ‘pain’ while the real subjects increased the voltage in order to teach them ‘to remember better’. Through a series of such experiments, Milgram proved that ordinary people's obedience to authority is much higher than what we all believe. Everyday people may be willing to kill in certain situations. Milgram’s real subjects were not aware of what they were involved in, and had to spend rest of their lives dealing with the fact that they were willing to inflict pain and death upon others, purely out of obedience. Although they participated voluntarily, their integrity was violated.
Milgram’s research demonstrated that we are all able to perform cruel acts if an authority figure assumes responsibility for them. This finding has been significant in our understanding of war crimes, among other things. Was Milgram right in carrying out his research? Would you have participated yourself?
More questions must be asked. Questions regarding research ethics concern the right to self-determination, to not have personal integrity violated, and to be protected from abuse.
It is not only about the right to not participate, but also the right to participate. It is about the right and the obligation to say no to authority figures and researchers, perhaps risking sanctions, ridicule or dismissal.
Ultimately, it also concerns how society utilizes new knowledge. Those who draw on this knowledge, at all levels, can and should pose questions regarding research ethics, and how we use the results of any unethical research.
