Source text - English The ‘protein folding problem’, i.e. understanding the detailed steps of the protein folding pathways is one of the most challenging topics in biochemistry. Combining theoretical with experimental research became indispensable for the investigation of the mechanisms in protein folding reactions.
Here, the biophysical characterization of designed ankyrin repeat (AR) proteins and the analysis of the experimental data by theoretical models are presented. Repeat proteins comprise repeating structural units that form linear multirepeat arrays. The non-globular elongated structure distinguishes them fundamentally from most studied proteins and makes repeat proteins interesting for folding studies.

Daher ist die Kombination von Theorie und Praxis unerlässlich, um die Mechanismen von Proteinfaltungsreaktionen zu erforschen.

Le Syndrome ptyalisme et dysphagie répondant au phénobarbital est un phénomène connu en médecine humaine mais peu décrit chez le chien.
Particulièrement représenté dans le groupe des Terriers et Spaniels (Jack Russel, Yorkshire, West Highland White terrier), il touche également de nombreuses autres races. Aucune prédisposition sexuelle n’a pu être mise en évidence jusqu’ici. L’âge moyen d’apparition serait de 3 à 9 ans, mais certains cas sont décrits chez des animaux très jeunes6 (10 semaines) ou âgés de plus de 10 ans1, 4-8, 10-12.
L’animal présenté ici est un Berger Allemand mâle de 7 mois et demi au diagnostic, présentant des signes cliniques depuis son adoption à l’âge de 6 mois. Tout comme les cas présentés par K.J. Gibbon6 et Schroeder et Berry11, le chien présenté ici est un berger allemand de moins de un an.
La clinique est variable et parfois frustre. L’atteinte des glandes salivaires est fréquente associée ou non à une nécrose de ces dernières, nécessitant parfois une sialoadenectomie. Lorsqu’elle touche les glandes salivaires zygomatiques, une exophtalmie avec kératoconjonctivite sèche secondaire peut être observée2, 4, 5. Les symptômes fréquemment rapportés sont une sialoadénite souvent associée à de la douleur, une hypersalivation, des mâchonnements, une douleur à la déglutition, un léchage fréquent des babines, des bâillonnements, une dyspnée. Des spasmes digestifs, des vomissements ne répondant pas au traitement symptomatique ou des régurgitations associées à un mégaoesophage sont également communément rapportées. En coutre, ces signes peuvent être accompagnés par une dysorexie, un amaigrissement, une dégradation de l’état général avec abattement1, 4-8, 10-12. Dans le cas présent, les glandes salivaires étaient hypertrophiée et légèrement douloureuse, et l’animal présentait une gêne importante à la déglutition. Une hypersalivation importante intermittente a été observée ainsi qu’une légère dyspnée pendant les crises (soufflement), accompagné de bruits respiratoires augmentés consécutifs à un excès de salive dans l’oropharynx.
L’étiopathogénie reste inconnue, cependant plusieurs mécanismes ont été proposés. L’hyperplasie de la glande salivaire peut-être liée à un processus loco-régional aigu, systémique ou être neurogène2, 4, 5, 12. La glande salivaire étant délimitée par une capsule non distensible, une hypertrophie glandulaire serait à l’origine d’un écrasement des vaisseaux sanguins, responsable d’un défaut de vascularisation et une nécrose des tissus 4, 5. C’est le cas de la sialométaplasie nécrosante des glandes salivaires, consécutive à l’ischémie focale des lobules salivaires conduisant à l’infarcissement de la glande. Elle est souvent unilatérale et des hypothèses neurogéniques sont avancées, notamment basées sur l’observation de ces phénomènes chez des sujets humains boulimiques et anorexiques1, 2, 7. La nécrose des glandes salivaires cependant semblerait être plus la conséquence de l’hypertrophie des glandes salivaires que la cause. Une autre entité est également rencontrée : il s’agit de la sialadénose1, correspondant à une augmentation uniforme bilatérale, peu douloureuse et non inflammatoire des glandes salivaires. A l’histologie, les lésions sont rares et sont une augmentation de taille des cellules associée à un œdème des tissus glandulaires2. Plusieurs hypothèses étiologiques (neurogénique, dystrophique, métabolique et/ou hormonale) sont avancées mais non démontrées à ce jour1,2, 10-12. Dans notre cas, l’échographie comme les cytoponctions des glandes n’ont pas mis en évidence de lésions tissulaires, au contraire des cas décrits par Mawby et al. ou encore Schroeder et Berry.
L’hyperplasie des glandes salivaires peut également être consécutive à un processus infectieux tel que la maladie de Carré. D’autres agents infectieux tels que les bartonnelles7, 10, Spirocerca lupi6, 11 ou encore la rage ont également été incriminés. La faible prévalence, associé au statut vacciné et à une clinique peu compatible de l’animal n’est pas en faveur de ces hypothèses.
Enfin une forme d’épilepsie « limbique » a été proposée, fondée sur le caractère intermittent des signes clinique, l’absence d’affections œsophago-gastriques systématiques et de lésions cytologiques des glandes salivaires, et sur la réponse au traitement par le phénobarbital4, 5, 6, 8, 10, 12. Le système limbique correspond à un ensemble de zones du cerveau jouant un rôle dans l’olfaction, la mémoire, la régulation des émotions, et participe au contrôle somatique et viscéral moteur (contrôle de la respiration, de la motilité et sécrétions intestinales, de l’activité cardiovasculaire et motrice, de la salivation, de la pilo-érection, motilité vésicale, motricité pupillaire). Il est constitué par le cortex limbique et les noyaux sous-corticaux en relation avec ce cortex et forme un cercle cortical autour de la surface médiale des hémisphères cérébraux. Sa régulation par le système autonome, endocrinien et somatique dépend du comportement émotionnel, d’où la possibilité d’exacerbation des signes cliniques lors de stress ou d’excitation4, 12. L’hypothèse d’une forme d’épilepsie particulière limbique est renforcée par la modification des tracés électroencéphalographiques observée dans une majorité des cas. Des accidents électriques (en fréquence et en amplitude) relevés notamment dans les lobes frontaux ou dans le système limbique ont été décrits9. Ces anomalies ne sont pas systématiques et les cas décrits répondent bien au phenobarbital. Dans notre cas, aucune anomalie oesophago-gastrique, des glandes salivaire et des nœuds lymphatiques régionaux n’a été mise en évidence. Le Berger Allemand fait partie des races prédisposées à l’épilepsie. Un électroencéphalogramme a révélé des anomalies d’activité cérébrale dans le lobe temporal gauche ainsi que dans le système limbique. Ces anomalies ont fortement diminué en fréquence et en amplitude trois semaines après la mise en place d’un traitement au phénobarbital renforçant l’hypothèse d’épilepsie « limbique ».
Le diagnostic se fait par exclusion. Les signes cliniques énoncés ci-avant, l’absence de réponse au traitement symptomatique des vomissements associée à des spasmes digestifs douloureux constituent des signes d’appel de ce syndrome, après exclusion des causes métaboliques et digestives.
Une numération formule révèle parfois une leucocytose neutrophilique témoin d’un processus inflammatoire actif. Une éosinophilie peut également être observée, décrite uniquement chez le Berger allemand. Les paramètres biochimiques sont normaux.
L’imagerie médicale permet d’exclure un mégaoesophage, ou la présence d’une masse compressive. Un transit baryté permet d’exclure toute anomalie fonctionnelle de la zone oro-pharyngée lors de la déglutition. Une endoscopie par voie haute permet de déceler oesophagite, gastrite, et de réaliser des biopsies stomacales et duodénales indispensables lors de vomissements chroniques ne rétrocédant pas au traitement. Dans ce syndrome, on peut observer une gastro-duodénite modérée, ne permettant pas généralement de justifier l’état clinique de l’animal. L’échographie de l’œsophage peut mettre en évidence des spasmes oesophagiens6 associés dans les formes chroniques à un épaississement de la paroi oesophagienne, notamment de la musculeuse. L’examen échographique permet également de visualiser des anomalies des glandes salivaires et des nœuds lymphatiques régionaux.
Une ponction avec analyse cytologique permet généralement de mettre en évidence un processus inflammatoire et/ou nécrotique. L’histopathologie sur biopsie ou exérèse de la glande constitue le diagnostic de certitude. Son utilité est actuellement mise en doute, puisque les glandes peuvent être normales macroscopiquement et histologiquement. L’exérèse des glandes salivaires ne devraient être réalisée qu’en cas de phénomène compressif important ou signes avancés de nécrose glandulaire. Elle n’est en revanche en aucun cas curative, contrairement à ce qui est observé chez l’homme2, 4. Dans le cas étudié, les examens complémentaires n’ont révélé aucune anomalie ou lésions permettant d’expliquer la clinique. L’hypertrophie des glandes salivaires est difficilement quantifiable à l’échographie. Elle permet cependant d’observer l’homogénéité du parenchyme glandulaire et des nœuds lymphatiques régionaux et d’apprécier les modifications pariétales de l’œsophage. Dans notre cas, le nœud lymphatique rétropharygien apparaissait de taille augmentée et hétérogène. La ponction de ce dernier n’a pu être effectuée en raison de sa proximité avec certaines structures nobles.
Le traitement repose sur l’administration de phénobarbital (4-6mg/kg/j en 2 prises orales). L’amélioration clinique est observée en quelques heures à quelques jours, renforçant l’hypothèse d’un mécanisme neurologique de type épilepsie limbique. Il a de plus été démontré qu’en cas d’arrêt trop rapide ou de non respect de l’administration des médicaments, les symptômes étaient susceptibles de réapparaître. Une mesure de la phénobarbitalémie peut être effectuée deux à trois semaines après la mise en place du traitement. Elle doit être comprise entre 15 et 45μg/ml. Dans notre cas, elle était à 11μg/ml. Cependant les symptômes ayant régressé, la posologie n’a pas été modifiée. Lors de persistance des signes cliniques malgré une concentration sérique normale, une association avec du bromure de potassium peut être proposée à une dose de 20-40mg/kg.
Un traitement à base de pilocarpine a également été proposé mais les résultats sont inconstants1. D’autres traitements avec des GABAergiques (gabapentine) ou le levetiracetam sont proposés dans le traitement de crises épileptiformes partielles mais n’ont jamais été testé utilisés dans ce genre de syndrome3.
Un traitement symptomatique (anti-inflammatoire et antiémétique) peut également être proposé en cas d’œsophagite et ou gastrite.
Le pronostic de ce phénomène est généralement bon. Une diminution progressive voire arrêt de la dose de phénobarbital à partir de 4 mois de traitement n’entraîne généralement pas de récidive. Cependant, un certain nombre d’animaux ne répondent pas au phénobarbital. Le pronostic annoncé doit donc être réservé en attendant la réponse au traitement. Dans le cas présenté ici, la disparition rapide des symptômes, associée à l’amélioration nette du tracé de l’EEG sont de pronostic favorable. Un nouvel EEG sera réalisé à 4 mois après la mise en place du traitement et un sevrage progressif sera proposé.

Sur un animal sain, les ondes électriques sont de hautes fréquence et de faible amplitude. Dans le cas présent, l’apparition d’accidents éléctriques caractérisés par une diminution de la fréquence des ondes et une augmentation de l’amplitude est anormale. Ces tracés sont compatibles avec une forme « d’épilepsie limbique »

Sous sédation, l’intervalle R-R devrait être constant. Le graphique montre des variations importantes de cet intervalle en fonction du temps, à priori consécutive à une activation anormale des systèmes parasympathique et orthosympathique compatible avec une forme d’épilepsie partielle.

Chez un animal sain sous sédation et non stimulé, l’intervalle R-R sur l’ECG devrait être constant. La courbe R-(R 1) en fonction de R-(R 1) en fonction du temps correspondrait alors à un point (constante). Chez le chien, l’arythmie sinusale respiratoire est à l’origine d’une modification des intervalles R-R sur l’ECG mais de façon régulière. Quatre points devraient alors être observés. Dans le cas présenté ici, le nuage de points est hétérogène démontrant une activité anormale de l’orthosympathique et du parasympathique.

Le nombre d’anomalies de tracé a fortement diminué. Certains accidents électriques subsistent néanmoins.

The salivation and dysphagia syndrome in response to Phenobarbital is a phenomenon known in human medicine but has not been described in the dog. It is especially represented in the group of terriers and spaniels (Jack Russel, Yorkshire, West Highland White terrier), but it also affects numerous other breeds. No sexual predisposition has been demonstrated so far. The average age of onset would be 3 to 9 years, but some cases have been described in very young animals6 (10 weeks) or in animals older than 10 years1, 4-8, 10-12.
The animal shown here is a male German Shepherd 7 1/2 month old at diagnosis, with clinical signs since its adoption at the age of 6 months. Just like the cases presented by K.J. Gibbon6 and Schroeder and Berry11, the dog presented here is a German Shepherd less than one year old.
The clinical symptoms are variable and sometimes frustrating. The damage of salivary glands is often associated or not with a necrosis thereof, sometimes requiring a sialoadenectomy. When it affects the zygomatic salivary glands, an exophthalmia with secondary dry keratoconjunctivitis may be observed 2,4,5. Frequently reported symptoms are sialoadenitis often associated with pain, hypersalivation, chewing, pain on swallowing, frequent licking of the lips, breathing difficulties and dyspnoea. Digestive spasms, vomiting unresponsive to symptomatic treatment or regurgitation associated with megaoesophagus are also commonly reported. Furthermore, these signs may be accompanied with dysorexia, weight loss, deterioration of the general condition and weakness 1, 4-8, 10-12. In this case, the salivary glands were enlarged and slightly painful, and the animal had a significant discomfort on swallowing. Intermittent important hypersalivation was observed and a slight dyspnoea during crises (wheezing), accompanied by increased respiratory noises resulting from an excess of saliva in the oropharynx.
The aetiopathogenesis remains unknown, however, several mechanisms have been proposed. Hyperplasia of the salivary gland may be related to a systemic acute loco-regional process, or be neurogenic 2, 4, 5, 12. The salivary gland is defined by a non-distensible capsule; glandular hypertrophy causes restriction of the blood vessels, which leads to insufficient vascularisation, and tissue necrosis 4, 5. This is the case of necrotizing sialometaplasia salivary glands, following focal ischaemia of salivary lobules leading to gland infarction. It is often unilateral, and neurogenic hypotheses have been advanced, especially based on the observation of these phenomena among bulimic and anorexic humans 1, 2, 7. Necrosis of the salivary glands, however, appears to be more the consequence of the enlargement of the salivary glands than the cause. Another entity is encountered: sialadenosis 1, corresponding to a uniform bilateral, painless and non-inflammatory enlargement of the salivary glands. Histologically, the laesions are rare and are an increase in size of the cells associated with glandular tissue oedema 2. Several aetiological hypotheses (neurogenic, dystrophic, metabolic and / or hormonal) have been put forward, but are unproven to date 1, 2, 10-12. In our case, neither ultrasound nor biopsy of the glands did reveal tissue damage, unlike the cases described by Mawby et al. or Schroeder and Berry.
Salivary gland hyperplasia can also be secondary to an infectious process such as canine distemper. Other infectious agents such as bartonnella 7, 10, Spirocerca lupi 6, 11 or rabies have also been implicated. The low prevalence together with the vaccination status and non-compatible clinical signs of the animal are not in favour of these hypotheses.
Finally, a "limbic" form of epilepsy was proposed, based on the intermittent nature of the clinical signs, the absence of systematic oesophago-gastric symptoms and cytological laesions of the salivary glands, and the response to treatment with Phenobarbital 4, 5, 6, 8, 10, 12. The limbic system corresponds to a set of brain regions that play a role in olfaction, memory, emotion regulation, and participates in somatic and visceral motor control (control of breathing, motility and intestinal secretions, cardiovascular and motor activity, salivation, piloerection, bladder motility, pupillary motor ability). It consists of the limbic cortex and subcortical nuclei in connection with this cortex and forms a cortical circle around the medial surface of the cerebral hemispheres. Its regulation by the endocrine and somatic autonomous system depends on emotional behaviour, hence the possibility of exacerbation of clinical signs during stress or excitation 4, 12. The hypothesis of a particular form of limbic epilepsy is enhanced by the modified EEG tracings observed in a majority of cases. Electrical accidents (frequency and amplitude) raised especially in the frontal lobes or in the limbic system have been described 9. These anomalies are not systemic and the cases described respond well to Phenobarbital. In our case, no oesophageal-gastric anomaly of salivary glands and regional lymph nodes was found. The German Shepherd is one of the breeds predisposed to epilepsy. An electroencephalogram showed abnormal brain activity in the left temporal lobe and in the limbic system. These anomalies had decreased significantly in frequency and amplitude three weeks after the introduction of a treatment with Phenobarbital, strengthening the hypothesis of "limbic" epilepsy.
The diagnosis is made by exclusion. The clinical signs set out above and the lack of response to symptomatic treatment of vomiting associated with painful digestive spasms represent warning signs for this syndrome after exclusion of metabolic and digestive causes.
A complete blood count may reveal neutrophilic leukocytosis confirming an active inflammatory process. Eosinophilia, described only in the German Shepherd, may also be observed. The biochemical parameters are normal.
Medical imaging permits to exclude a megaoesophagus, or the presence of a compressive mass. A barium enema permits to exclude any functional abnormality of the oro-pharyngeal area during swallowing. High channel endoscopy allows to detect oesophagitis, gastritis, and to carry out stomach and duodenal biopsies essential during chronic vomiting not responding to treatment. In this syndrome, a moderate gastro-duodenitis may be observed, generally not allowing to justify the clinical condition of the animal. Ultrasound of the oesophagus can highlight oesophageal spasms 6, in chronic forms associated with a thickening of the oesophageal wall, in particular the muscular wall. The ultrasound examination can also show abnormalities of the salivary glands and regional lymph nodes.
A puncture with cytological analysis is generally used to confirm an inflammatory and / or necrotic process. Histopathology on biopsy or excision of the gland constitutes a sure and certain diagnosis. Its utility is currently in doubt, since the glands may be macroscopically and histological normal. The removal of the salivary glands should only be performed in case of significant compression phenomenon or advanced signs of glandular necrosis. It is, however, by no means curative, unlike what is observed in humans 2, 4. In the case studied, complementary tests revealed no abnormality or injuries to explain the clinical signs. Salivary gland hypertrophy is difficult to quantify by ultrasound. However, it allows us to observe the homogeneity of the glandular parenchyma and regional lymph nodes and to appreciate the parietal modifications in the oesophagus. In our case, the retropharyngeal lymph node appeared of increased size and heterogeneous. Puncture of the latter could not be performed because of its proximity to some sensitive structures.
The treatment is based on the administration of Phenobarbital (4-6mg/kg/d 2 oral doses). Clinical improvement was observed within a few hours to a few days, reinforcing the hypothesis of a neurological mechanism of the limbic epilepsy type. It was further shown that when the treatment was discontinued too early or when there was non-compliance with medication administration, the symptoms were likely to reappear. A measurement of the Phenobarbital administration can be performed two to three weeks after initiation of treatment. It should be between 15 and 45μg/ml. In our case, it was 11μg/ml. However, the symptoms decreased, but the dosage was not modified. With persistent clinical signs despite normal serum concentration, combination with potassium bromide may be proposed at a dose of 20-40mg/kg.
Treatment with pilocarpine has also been proposed, but the results are inconsistant 1. Other treatments with GABAergic molecules (gabapentin) or levetiracetam are proposed in the treatment of partial epileptic seizures but have never been tested for this kind of syndrome 3.
Symptomatic treatment (anti-inflammatory and anti-emetic) can also be offered in case of oesophagitis and / or gastritis.
The prognosis of this phenomenon is generally good. A gradual reduction or discontinuation of the dose of Phenobarbital after 4 months of treatment does not generally result in recurrence. However, a number of animals do not respond to Phenobarbital. The announced prognosis must therefore be reserved pending the response to treatment. In the case presented here, the rapid disappearance of the symptoms together with the improvement of the EEG pattern allow a favourable prognosis. A new EEG will be performed at 4 months after initiation of the treatment and a gradual withdrawal will be offered.

In a healthy animal, electrical waves are high frequency and low amplitude. In this case, the occurrence of electric accidents characterized by a decrease in the frequency of the waves and an increase of the amplitude is abnormal. These plots are consistent with a form of "limbic epilepsy"

Sedated, the R-R interval should be constant. The graph shows significant variations in this range depending on the time, a priori consecutive to abnormal activation of the parasympathetic and orthosympathetic systems compatible with a form of partial epilepsy.

In a sedated and unstimulated healthy animal, the R-R interval on the ECG should be constant. The R-(R 1) curve as a function of R-(R 1) as a function of time would then correspond to a point (constant). In dogs, the respiratory sinus arrhythmia is causing a modification of the R-R intervals on the ECG but on a regular basis. Four points should then be observed. In the case presented here, the point cloud is heterogeneous showing abnormal activity of the orthosympathetic and the parasympathetic.
