Source text - English Tau is the major microtubule associated protein (MAP) of a mature neuron. The other two neuronal MAPs are MAP1 and MAP2. An established function of MAPs is their interaction with tubulin and promotion of its assembly into microtubules and stabilization of the microtubule network. The microtubule assembly promoting activity of tau, a phosphoprotein, is regulated by its degree of phosphorylation. Normal adult human brain tau contains 2–3 moles phosphate/mole of tau protein. Hyperphosphorylation of tau depresses this biological activity of tau. In Alzheimer Disease (AD) brain tau is three to four-fold more hyperphosphorylated than the normal adult brain tau and in this hyperphosphorylated state it is polymerized into paired helical filaments ([PHF) admixed with straight filaments (SF) forming neurofibrillary tangles. Tau is transiently hyperphosphorylated during development and during anesthesia and hypothermia but not to the same state as in AD brain. The abnormally hyperphosphorylated tau in AD brain is distinguished from transiently hyperphosphorylated tau by its ability (1) to sequester normal tau, MAP1 and MAP2 and disrupt microtubules, and (2) to self-assemble into PHF/SF. The cytosolic abnormally hyperphosphorylated tau, because of oligomerization, unlike normal tau, is sedimentable and on self-assembly into PHF/SF, loses its ability to sequester normal MAPs. Some of the tau in AD brain is truncated which also promotes its self-assembly. Tau mutations found in frontotemporal dementia apparently promote its abnormal hyperphosphorylation. Thus, the AD abnormally hyperphosphorylated tau (1) is distinguishable from both normal and transiently hyperphosphorylated taus, and (2) is inhibitory when in a cytosolic/oligomeric state but not when it is self-assembled into PHF/SF. Inhibition of abnormal hyperphosphorylation of tau offers a promising therapeutic target for AD and related tauopathies.
Translation - Korean 타우는 성숙한 뉴런의 주요 미세소관 결합 단백질(microtubule associated protein, MAP)이다. 신경 MAPs의 다른 두 종류에는 MAP1과 MAP2가 있다. MAPs의 기능은 튜불린(tubulin)과의 상호작용, 튜불린과 미세소관 조립 촉진, 미세소관 네트워크의 안정화이다. 인단백질인 타우의 미세소관 결합 촉진 활동은 인산화 정도에 따라 조절된다. 정상 성인 뇌의 경우 타우 단백질 1몰당 2-3몰의 인을 포함한다. 타우의 과인산화는 이러한 타우의 생물학적 활동을 억제한다. 알츠하이머병(Alzheimer Disease, AD)에 걸린 뇌의 타우는 정상 성인 경우보다 3~4번 더 접혀 과인산화되고, 이러한 과인산화 상태에서 타우는 쌍 나선형 필라멘트(paired helical filaments, PHF)로 중합하며 직선 필라멘트(straight filaments, SF)와 결합해 신경섬유 엉킴(neurofibrillary tangles)을 형성한다. 타우는 발달 과정과 마취 및 저체온 상태에서 일시적으로 과인산화되지만 AD의 뇌 상태와 같은 농도까지 과인산화되지는 않는다. AD 뇌의 비정상적으로 과인산화된 타우는 일시적으로 과인산화된 타우와 다음 능력에서 구별된다. (1) 정상 타우, MAP1, MAP2 격리 및 미세소관 붕괴, (2) PHF/SF로 자기 조립. 올리고머화(oligomerization)로 인해 비정상적으로 세포질 과인산화된 타우는 정상 타우와는 달리 침전 가능하고, PHF/SF로 자기 조립하여 정상 MAPs 격리 능력을 잃게 된다. AD 뇌의 일부 타우는 길이가 줄어 드는데 이것 역시 자기 조립을 촉진시킨다. 전측두엽 치매에서 발견되는 타우 돌연변이는 비정상적 과인산화를 촉진시킨다. 따라서 AD로 비정상적으로 과인산화된 타우는 (1) 정상 및 일시적으로 과인산화된 타우와 구별이 가능하고, (2) 세포질/올리고머 상태에 있을 때 억제성을 띠지만 PHF/SF로 자기 조립할 때는 그렇지 않다. 비정상적으로 과인산화된 타우의 억제성은 AD 및 관련 타우 병증에 있어 가능성 있는 치료 대상을 제공한다.
English to Korean: Therapeutic Pilates-based Exercise Progressions for Hyperkyphosis General field: Other Detailed field: Sports / Fitness / Recreation
Source text - English Age-related hyperkyphosis affects up to 40% of adults over age 60. Characterized by a Cobb angle of kyphosis greater than 40°, this excessive forward curvature of the thoracic spine limits mobility. As the curvature increases, mechanical restrictions can impair respiratory function and balance, leading to issues such as pneumonia, falls and fractures and even depression caused by low-life satisfaction.
A promising study shows that targeted spine-strengthening exercises and posture training are effective rehabilitation modalities for decreasing hyperkyphosis in older adults. The study cites spinal extensor strengthening, spinal mobility exercises, and other posture-focused exercises as successful in reducing both radiographic and clinical measures of hyperkyphosis by up to 4°.
Goals of the exercises
1. Strengthen spinal extensors and integrate into posterior slings/chains of the body
2. Improve spinal mobility and stability
3. Enhance postural awareness and control in a variety of positions, with gravity acting on the body in varying planes, with and without load
Phase I
• Breathing with Resistance Band around lower rib cage to facilitate 3D breath pattern and diaphragmatic breath. Discuss breath mechanics and need for rib cage excursion in posterolateral direction. Beginning facilitation of rib cage and spinal mobility. Relate inhale to spinal extension and exhale to spinal flexion
• Scapula isolations seated with Foam Roller along spine (pro-retraction, elevation, depression) educate on positioning of scapula in neutral, mobility of scapula for healthy spinal and shoulder mechanics. Relate coupled movements of scapular retraction/spinal extension, scapular protraction/spinal flexion
• Supine head positioning with pads to achieve neutral cervical spine, positioning of rib cage and pelvic neutral. Use peripheral movement such as arm scissors and circles to challenge holding this neutral spinal alignment. Illustrate the abdominal musculature origin and insertions as the connection which relate the rib cage to the pelvis. Often with a hyperkyphotic posture there will be a thoracolumbar region hypermobility
• Bilateral closed-chain hip extension prone over Arc Barrel or BOSU® with head down resting on backs of hands. Monitor cervical and thoracolumbar region as client extends hip and lifts knee off mat. Teaching spinal stability with proper hip extension recruitment pattern. Progress to single leg and then to open chain
• Log roll with Foam Roller, side lying. Cue the relationship and stability of head, rib cage, and pelvis with a 45-degree forward roll and return to side lying—this facilitates the multifidus
Translation - Korean 60세 이상 성인의 40% 가까이 노화로 인한 척추 후만증을 겪습니다. Cobb 각도상 후만이 40°보다 큰 것을 특징으로 하는 이러한 흉추의 과도한 만곡은 가동성을 제한합니다. 만곡이 증가할수록 기계적 제한이 호흡 기능과 균형을 손상시키고, 폐렴, 추락 및 골절, 삶의 만족도 저하로 인한 우울까지 일으킬 수 있습니다.
한 흥미로운 연구에서 척추 강화를 목표로 한 운동과 자세 훈련이 연령대가 높은 성인의 척추 후만증 감소를 위한 재활 치료에 효과적임을 보여주었습니다. 해당 연구에 따르면 척추 신전근 강화, 척추 가동성 운동, 기타 자세 집중 운동으로 척추 후만증이 영상 의학 및 임상적 척도상 4°까지 감소하는데 성공했습니다.

1단계
• 3D 호흡 패턴과 횡격막 호흡을 익힐 수 있도록 저항 밴드(Resistance Band)를 흉곽에 두르고 호흡합니다. 호흡 역학과 흉곽의 측면 및 후면 운동의 필요성을 논의하세요. 흉곽 및 척추 가동성을 도와줍니다. 흡기와 척추 신전, 호기와 척추 굴곡을 연결해 보세요.
• 폼롤러(Foam Roller)에 앉아 척추를 따라 견갑골을 분리하는 운동(전인과 후인, 거상과 하강)은 건강한 척추 및 견갑골 구조를 위해 중립 상태의 견갑골 위치와 견갑골 가동성을 가르쳐 줍니다. 견갑골 후인/척추 신전, 견갑골 전인/척추 굴곡을 결합한 운동을 연결해 보세요.
• 패드에 머리를 대고 누워 경추의 중립을 확보하고, 흉곽 및 골반을 중립 자세에 둡니다. 척추의 중립 정렬 유지에 도전이 되는 암 시저(arm scissors), 암 서클(arm circle)과 같은 사지 운동을 이용해 보세요. 흉곽 및 골반으로 연결되는 복부 근육들의 기시 및 종지를 그려 보세요. 척추 후만증의 경우 종종 흉요추 부위에 과가동성이 있을 수 있습니다.
• 아크 배럴(Arc Barrel)이나 보수(BOSU)®에 엎드려 머리를 손등에 대고 두 다리 함께 움직이는 닫힌 사슬 힙 익스텐션(hip extention) 동작을 합니다. 고관절을 신전하고 무릎을 매트에서 들어올릴 때 경추 및 흉요추 부위를 살피세요. 적절한 힙 익스텐션 패턴으로 척추 안정성을 가르칩니다. 싱글 레그(single leg), 열린 사슬 운동 순서로 진행합니다.
• 폼롤러(Foam Roller)에 옆으로 누운 자세로 로그롤(log roll) 동작을 합니다. 앞쪽으로 45도 굴러갔다 돌아오는 동작으로 머리, 흉곽, 골반의 관계와 안정성을 가르쳐 주세요. 이 동작은 다열근 활성을 돕습니다.
