When instructed to participate in activities and skill practices, please locate these materials at the back of the relevant topic tab in the Guide
As I mentioned earlier, you can also find a blank Individual Action Plan for each topic at the back of each topic tab

Il me semble qu'il s'agit de documents papier, non ? Classeur ?Auquel cas, on sépare bien les différentes rubriques (ou comme on voudra bien les nommer) par des intercalaires, mais je ne pense pas que les exercices, modèle de plan d'action, etc. soient au dos de l'intercalaire... plutôt à la fin de la rubrique.
Je traduirais ainsi
"at the back" à la fin de la rubrique
"under " sous (ou dans) la rubrique
"behind" ... une rubrique où vous pouvez insérer...
