this is from an article on thyroid operations

"Una de las complicaciones más temidas en el postoperatorio inmediato es la hemorragia y el desarrollo de hematoma transfixiante que puede comprometer la vida del paciente. "

"En estos casos, el drenaje no puede prevenir los hematomas transfixiantes ya que la formación de coágulo ocurra con probabilidad y el drenaje será bloqueado"

Google has been of little help this time, and the closest I have got so far is in the French Kudoz "through and through", which doesn't really work, I don't think - "through and through hemotoma".

Any experts out there this evening, please?

FWIW In another veterinary text, totally different authors from my post 1 year ago, I have:
"Se han descrito muchas variantes de la OHE, incluyendo los abordajes por el flanco y laparoscópicos y el uso de grapas, bisturíes ultrasónicos, sistemas de sellado de vasos, ***ligaduras transfixiantes*** o nudos de Miller."
Just another example of its use in another context.

FWIW I think "deep-tissue" might be close based on a text I'm translating:
In the context of eyelid lacerations in dogs, "...pueden ser un motivo de intervención urgente si son transfixiantes (afectan a todo el espesor palpebral)".

hematomas associated with thyroid surgery I did some searching of hematomas in thyroid surgery, and the hematomas were most often referred to as "neck hematoma" or, much less often, "rectus sheath" - nothing remotely similar to your word.
Nor does your word appear in the list of hematomas in Dorland.

I think the author refers to patients that may develop a tension hematoma with respiratory obstruction after thyroid surgery despite the presence of a drain .

Potential risks with thyroid surgery

Thyroid surgery can offer significant health benefits. However, it also involves some risks of which patients should be aware. Less than 2 percent of people who undergo thyroid surgery experience these complications, according to the American Thyroid Association.

Thyroid surgery includes risk of damage to the recurrent and external laryngeal nerves that control the vocal cords. This can result in hoarseness or a weakening of the voice.

Surgery also can damage the parathyroid glands, which control the level of calcium in the blood. Damage to the parathyroid glands may require the patient to take calcium medication to ensure that levels of calcium in the bloodstream remain normal. Patients are urged to contact their physician if they experience tingling or twitching after the operation. This reaction can indicate that a person’s calcium level is low, which may occur if the parathyroid gland is damaged.

In some cases, bleeding occurs that may lead to acute respiratory distress. Rarely, postoperative bleeding can cause an expanding hematoma (swelling of blood) that may constrict the airway. Sudden swelling in the wound area may indicate bleeding.

Surgery may result in a visible scar on the neck. In addition, thyroid surgery involves all of the usual risks involved with any surgery, including infection and adverse reaction to anesthesia. Redness and drainage from the wound area may indicate infection.
