Czech Bus/Financial Finance (general) Payslip

I am translating a payslip and this is one of the column headers. I am trying to decypher all the abbreviations so I know what they stand for in the Czech in order to translate into English.

Radovan Pletka Jan 11, 2010: Maybe you can call the company which issued the payslip, it should be clear from the payslip what company it is - I am sure the client would not mind and if you ask for explanation of your husband payslip (smile), they will surely help you. And because it looks like a weird mix of CZ and ENG, asking seems like a proper and prudent way to handle this issue. And if the customer is an agency, they shoudl be more supportive as well.
