The form you describe is very odd for two reasons.

First, the word اسـتئذان simply means a request for permission. Any request submitted by an employee for time off, whether for two hours or two weeks, is a request for a permission to be off. The word by itself does not denote a request for a short, rather than long, time off.

Second, an administrative request for leave should state how the off time will be counted. A request for two hours off could still be counted as vacation, so having one box for "vacation" and the other for a short leave makes no sense. The typical request for leave has boxes for "vacation" (also called "annual leave), "sick leave," "maternity leave," "unpaid leave," and "other" (the last category covers jury duty, etc.).

To be true to the intent of the form, regardless of whether it makes sense or not, you need a word that simply denotes a brief time off (an hour to a few hours). I think the word "break" gives that sense.

This, of course, does not translate the word اسـتئذان, but it translates the intended meaning. Translating the word itself will produce a completely meaningless form.
