Я всегда думала, что это про наркотики, однако после просмотра нескольких форумов выясняется, что эта строчка имеет отношение к сайентологии, т.к. Леонард Коэн одно время ей увлекался и сам признал это в одном из интервью.

It was a Scientology reference. I looked into a lot of things. Scientology was one of them. It did not last very long.

Sometimes only the artist knows what he is referring to and that seems to be the case here. I would guess that maybe this person was on drugs or maybe drank a lot and the singer is asking him if he has stopped using drugs or alcohol. But that is just a guess.

For the rest of the song I guess that meaning about giving up drugs or similar would fit.

My opinion is that L. Cohen was speaking about drug detox. The original worlds sounded like that :

Did you ever "stop using"... and he eventually changed them (visibly for the rhyme) into Did you ever "go clear".
