Another suggestion.

´Vilkår´ is often translated as ´terms and conditions´ in contracts, so I might or might not add betingelser, depending on the style and register of the rest of the document.

Aftalevilkår?
Probably not in this case.

I would like to see what the Danish native speakers suggest!
