RESPONSABLE on a hospital admission sheet
This is a heading on a hospital admission sheet, below the patient information and guardian information, the next section is "RESPONSABLE". It looks like Payor information because the sections under this heading are Type: business, Name: (name of a company that sounds like it's related to health care), then there are two more fields, "Nit" and "Telefono" that are redacted, presumably because they could be used to identify the patient, and finally a field labeled "tarifa", which is also redacted. It's from Latin America, but I don't know the country.

I think since you are not sure if this is a doctor or insurance comapny l'd keep it ambiguous in the translation as well. It most often is the "responsible party" that is to be billed but it could in theory apply to admitting physician as well

Either the doctor in charge, or the insurance company, I would say..
I would think it would be about the doctor in charge (Responsable) or maybe the insurance company since you say that section has payor information.
Discussion
Then there is the "RESPONSABLE" section. Everything is redacted, except the type, which is "business", and the name, which I have looked up and it's a Colombian provider of health care services. I'm used to seeing "Responsible party" in English as the name of the person or insurance company that's going to pay the bill. But in this case it could be the admitting physician, except that there is no doctor's name, only the company name. The document goes on for 4 pages, including all the case history, but all doctors' names are redacted as well as patient identifying information.
