Nothing comes after PORT. While the previous sentence is capitalized, UHCD and PORT are indented and in larger font than the sentence. It isn't a navy form -- hospital is Groupe hospitalier Pitié-Salpêtrière. Thank-you for trying!

Since everythin else is capitalized, I am not sure this is an acronym.
What comes immediately after?
It could mean patient is transferred or discharged and carrying a medical device (Holter for example).
It should be médicale with an E btw.
Could this be a Navy form?

In some UHCD patients are asked to wear an identification bracelet, which they can refuse to wear. However, this would not be in capital letters as in your original.
Which hospital in concerned? Might help with fishing around for info.
