I have only found "modal fee" in documents related to transportation, but it has nothing to do with my document, which is a financial statement for healthcare insurance.

el termino en realidad se aplica a las cuotas que se pagan de acuerdo al porcentaje volumen.

un "fee" tambien se puede entender como "honorarios" que se pagan a un profesional o que se pagan por un servicio. Pero el uso de "modal" me indica que estamos hablando de un tipo de "cuota ajustable". Creo que este termino se acerca mas a la idea original.
