How would we say this in the UK if we got some dodgy packages? Correspondence usually starts off fairly politely: "We would like to point out that...", or "Please note that five packages were soaking wet", etc., the implication being that some remedial action is very much expected.

Is this the recipient notifying the sender? If so, you could say something like "This consignment was three pallets short - please rectify".

The term 'reservations' is widely used in various industries for items that do not meet specifications. The item is not fully rejected but the 'reservations' must be remedied or lifted before that item is accepted completely.

agree Nikki Scott-Despaigne : This is the terminology I am more familiar with (UK) from my experience in merchant shipping and more generally, down to small parcels. However, "delivery exceptions" seems to be gaining ground, with regard to smaller units at least, see UPS and FedEx.
