This is how I would head the document.
As your additional context (thank you!) shows, this is a form designed for use when there is clearly some risk that the goods are not in the required condition.
The "Eimgangsvorbehalt" is "reservation or proviso" (placed on the "Anlieferung"), or the "delivery of goods".
Many years ago, just for interest's sake, the usual procedure was to sign a delivery note for consignments which might have been damaged etc., "Goods not examined", but that provision has long since been replaced.

: The asker has a table to translate, and this is the sort of notice you would find leading to the delivery bays, but not in a table! I'd suggest you wait until more context is forthcoming, then have another look!.

Well, I'm not in the bizz of begging Asker for more context (which seems to be the most common problem in 9 out of 10 cases on this forum). One gets a translation conform the term given in its context. Lack of such cannot be blamed on answerer.

... hieße in etwa soviel wie Wareneingang unter Vorbehalt, was etwa bedeutungsgleich wäre. Gemeint ist, dass die reine Lieferung nicht ausreicht. Der eigentliche Wareneingang, sprich das Buchen des gelieferten Materials in den Lagerbestand erfolgt erst nach einer Qualitätsprüfung o.ä. Erst dann gilt die Ware als angenommen...

In certain contexts an "Eingangsvorbehalt" means subject to actual delivery / supply ...
But we are guessing here without more information.
