Hi is this referring here to the specialist doctor who is dealing with the patient with the chronic condition?? What is the purpose of this card / when is it used?? Many thanks in advance

The Arabic is not right. It should have been طبيب الاختصاص أو الطبيب المختصّ
I think the best way to render it is to say the specialized doctor, they mean that he is the only one who can change the medication card for the patient.

From the text, it seems that the patient with this chronic disease holds a medication card. All the treatment medications should not be changed except by the specialized physician who is in charge in that public clinic. Not sure about the process of using this card.

Many thanks for confirming that. Does the patient, the holder of this card, take this to the pharmacy to receive free/subsidised (by the state) medication for their condition which seems to be diabetes in this case??
