English to Spanish Law/Patents Medical: Health Care Consent form

Hello, everyone.

I'd like to ask for your help with this term into Spanish.
It appears on a consent form: "I, the undersigned, a client, guardian, guardian advocate, or health care surrogate/proxy hereby authorize..."

I did some research and found that it's not the same as guardian. In Guardian Advocacy there is no determination of incapacity required, a certain level of incapacity is presumed for a person with a development disability.
This is only available in Florida for developmental disabilities.
