I am not sure what "Environmental factors also contextualize where older persons live" means.

@Yasutomo I don't know about US English, but in UK English those would be called "gang members", not "gangsters", the latter being serious, violent criminals.

@B D Finch Just for the record, gangster here means not like mafias or drug cartels, but street gangs you see in American movies which they consist of neighborhood boys.

Contextualize here means to place something or someone in a particular context. Context in this case means "1.The surroundings, circumstances, environment, background or settings that determine, specify, or clarify the meaning of an event or other occurrence. "

So, environmental factors have a great influence on where older persons live. Suppose they lived in a slum full of gangsters, criminals and other anti-social people, these older people might get intimidated and may never leave their rooms, being isolated and fear for their lives. But if they lived in an environment or facility with the same age group, they might be happier than living in a slum and talk about common topics and share experiences with others they had until they grew old.

Therefore, in this question's case, the older persons' both mental and physcial health would greatly be influenced by where they live.

I'd paraphrase it thus:

The environment in which older people live determines patterns of inequity and, when better understood, informs multi-sectoral policies that can improve their ability to function.

I have no idea what 'multi-sectoral policies' are.

Places where older persons live have to be seen in the context of environmental factors. However, this is dreadfully vague. It could mean e.g.

* the factors mentioned by Yasutomo (criminal activity etc., though successful gangsters generally don't live in slums);
* whether they live in a country area with clean, fresh air, healthy veg from the garden, but no public transport or support networks;
* environmental pollution could have a serious effect on their health;
* etc.
