Les études se déroulent en trois cycles… un premier cycle de trois ans dit « de formation générale », d'un deuxième cycle de trois ans dit « de formation approfondie », et d'un troisième cycle de trois à cinq ans dit « d'études spécialisées »…
Deuxième cycle… Le diplôme de formation approfondie en sciences médicales… confère le grade de master.
Troisième cycle… sous la responsabilité d'un « senior », l’interne est toujours en formation puisqu'il ne peut pas exercer sans compléter son internat (la France reste un des rares pays à considérer les internes comme des étudiants)… Il soutient un mémoire portant sur un sujet de sa spécialité qui peut être confondu avec sa thèse d'exercice lorsque celle-ci est soutenue lors de la dernière année d'internat. Cette dernière lui confère, après soutenance avec succès, le diplôme d'État de docteur en médecine.

Le « rang » n’est donc pas le même. Et si cela avait été le cas, bien que certaines de ces personnes puissent être collègues [= celui ou celle qui, à l'intérieur d'une institution… est revêtu de la même fonction… que d'autres] ensemble, ce sont des confrères et consœurs [personnes faisant partie d'un corps social, considérée par rapport aux autres membres de ce corps]. Leur point commun se situe dans leurs études post-doctorales ou, à la rigueur, dans le fait d’être boursiers. Au plan d'un ordre/d'une association professionnelle, le statut est différent.

Un "fellow" est un professionnel (médecin, chirurgien, chimiste, etc.) qui poursuit des études post-doctorales ou détient le diplôme d'études post-doctorales et le titre - souvent "Fellow", non traduit - que son ordre/association professionnelle lui confère de ce fait. C'est aussi (dans la plupart des cas et souvent en médecine) un "boursier", le "titulaire d'une bourse de perfectionnement", etc. Cf. Termium, GDT et... les séries médicales télévisées et Web, depuis Grey's Anatomy jusqu'à Saving Hope en passant par Dr House (dont la post-synchro française fait mal chaque fois que le mot "House" est prononcé, mais ça, c'est une autre histoire! ;-)
