"para acceder al documento electrónico original, introducir el..."
means "in order to access the original electronic document, (please) enter the (national) ID number of the person concerned and the process number in the following website [I believe "address" here is not necessary].
In Argentina "número de documento" refers to your ID number, unless when it makes reference to a specific document, which is not the case here.
I'm from Argentina and I've translated quite a number of this.
