Under the handwritten entry notes are some medical observations. Am unsure what this is (it may be that it is DCRD - handwriting hard to decipher):

Denise Pavao Nov 6, 2018: I agree with Jacqueline - DLPD is a strong candidate to go between HTA and DBT2. Handwritten content is always such a challenge!
Alison Wedley (asker) Nov 6, 2018: Yes Jacqueline, that is what somebody else has suggested. Looking at it again, it could well be an L.
Jacqueline Lamb Nov 6, 2018: DLPD? I'm wondering if the handwritten "C" is actually an "L". If so, it could be referring to dyslipidemia, which is often mentioned in the same part of the report as hypertension and diabetes.
