La violencia contra los adolescentes y los niños en el hogar sigue siendo un problema constante, y los adolescentes tienen limitadas oportunidades de educación y comprensión respecto a la salud sexual y reproductiva, lo que resulta en un embarazo adolescente y un matrimonio precoz que ubica al país [nombre del país] con el 12.45% de mujeres que tienen hijos menores de 18 años.
Quería utilizar el verbo "rank" pero en este caso no podría porque se refieren a ubicación con respecto a un porcentaje determinado.
Alguna sugerencia, aparte de la traducción literal de "... which/that places the country with 12.45% of women..."

Clearly you're right about the last part: "menores de 18 años" must refer to the mothers, not the children. However, I think the meaning is "12.45% of women have children [when they are] under the age of 18" (el 12.45% de mujeres tienen hijos cuando son menores de 18 años/antes de cumplir los 18 años), rather than "12.45% of women under the age of 18 have children" (el 12.45% de mujeres menores de 18 años tienen hijos), which is not the same thing. I think this is why "menores de 18 años" has been put after "tienen hijos". If I'm right, I would suggest you might say "have children before the age of 18", which is not ambiguous. Though it might be a good idea to check this with the client.

En español, la idea que indica la redacción (en mi humilde opinión) “..que ubica a el país xxx con el 12.45% de mujeres que tienen hijos menores de 18 años..” es que hay un 12.45% de mujeres que tienen hijos cuya edad (la edad de los hijos) es menor de 18 años; sin embargo, más bien me parece que la idea aquí es que hay mujeres que dan a luz antes de cumplir los 18 años.
Aunque estoy consciente que no es parte de la pregunta que hice; sin embargo, solo quería expresar mi opinión al respecto. De todos modos, tendría que consultar con el cliente.
