rodete septal basal

Spanish to English Medical Medical: Cardiology Echocardiogram
Hi all,

This is the discussion of the results of an M-mode echocardiogram, not a lot of context as the whole document is only 150 words.
My problem is how to translate "rodete" - is it something to do with "cushion" (endocardial cushion), or should it be "ring"? Or something else? Any help appreciated.

"VI no dilatado.

Pej., en referencias previas;
Distinguishing ventricular septal bulge versus hypertrophic cardiomyopathy

O también;
An isolated hypertrophy of the basal segment of the interventricular septum protruding into the outflow tract of the left ventricle (LV), named ventricular septal bulge (VSB), (...)

¡Saludos, Neil!

I came across articles that distinguished between bulge and hypertrophy based on size. 13 mm was right on the limit in a few cases.... ;@)
Chema Nieto Castañón Feb 6, 2019:
Both bulge and hypertrophy refer to the same thing -with different names;
sigmoid septum, ventricular septal bulge, upper septal hypertrophy, basal ventricular septal hypertrophy... As far as I understand, and in spite of the apparent differences, they all convey the idea of rodete septal / septo sigmoideo / hipertrofia del segmento subaórtico del septo interventricular. Diameter though does not convey the idea of (discrete) hypertrophy that the original seems to be trying to express.
Neil Ashby (asker) Feb 6, 2019:
Okay Chema, thanks for your help. I've put "bulge" and included a note to the client saying that given the value (13 mm) it's probably bulge but could also be "hypertrophy" or "diameter". Cheers.
Chema Nieto Castañón Feb 5, 2019:
And so, answering your previous question, I would say that the original expression does not refer just to basal septal diameter but to basal [upper] septal hypertrophy (basal interventricular septal hypertrophy) of 13 mm.

Neil Ashby (asker) Feb 5, 2019:
Do you think it could simply refer to the diameter - basal septal diameter?
Neil Ashby (asker) Feb 5, 2019:
Nope, I hadn't considered or looked into "halo"... I will now.

Indeed, I can't find anything for basal septal ring either!

Neil Ashby (asker) Feb 5, 2019:
Thanks Liz, so you think it might be "ring", I can't find much for "basal septal ring", that's why I'm asking in Kudoz. However, there are a few hits for "septal ring"... so probably on the right lines, thanks for your input.

ventricular septal bulge

Hola Neil,

A localized hypertrophy of the subaortic segment of the ventricular septum has been frequently described in elderly persons, and variously termed subaortic ventricular septal bulge (VSB), sigmoid-shaped septum, localized or discrete upper septal hypertrophy. It has been considered mainly as an incidental finding associated with older age and hypertension, although some authors have suggested that it may be part of the spectrum of hypertrophic cardiomyopathy, may determine some degree of left ventricular (LV) obstruction, which in turn may potentially cause symptoms such as syncope and dyspnea on effort
(...)
According to a combination of criteria applied in previous studies, the presence of subaortic VSB was defined as a proximal focal area (within the first third of total septal length) of localized septal hypertrophy with a dune-like structure protruding in the LVOT, a thickness ≥ 13 mm in men and ≥ 12 mm in women, and more than 50% greater than the thickness of the septum at its mid-distal-point
https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4495884/

Dynamic obstruction may also occur in elderly patients with concentric left ventricular hypertrophy (LVH) due to chronic hypertension; in these patients the aorta shifts to the right creating a basal interventricular septal bulge. This septal bulge can cause SAM of the anterior mitral valve into the LVOT resulting in dynamic LVOT obstruction.
