@Jane Yes, I suppose I could ask the client for a screenshot. Otherwise, I'll probably use "relevant" as a search for "check relevant box" gets 84,000 hits, whereas replacing relevant with "pertinent" brings the results down to a couple of hundred, and "check the corresponding box" gets over 64,000 hits.

Whether you need to translate 'correspondiente' or not probably depends on the layout of the form/page and whether it is obvious which box you have to tick or not. Of course you may not have that information.... in which case I would probably translate it - just to be safe.
