La escala o índice de Karnofsky (KPS) consiste en un escala numérica del 0 al 100, muy empleada en oncología para expresar de forma reducida el estado general de salud y la calidad de vida de un paciente.

El Índice de Karnofsky es una herramienta de valoración funcional, también llamada Escala de Karnoksky, que permite clasificar a los pacientes según su deterioro y sus capacidades para realizar tareas cotidianas. También puede servir para medir la evolución de los pacientes, para predecir la mortalidad, para tomar decisiones clínicas y para valorar la eficacia de distintas terapias.

La Escala de Karnofsky va del 0 al 100, cuanto más elevada sea la puntuación significa que el paciente tiene una mayor capacidad para desenvolverse en las actividades del día a día.

La Escala de Karnosfsky1 muestra el valor asignado a cada paciente de acuerdo con su grado de enfermedad y estado funcional. Es una tabla descendiente que va de la normalidad a la muerte. Así pues los pacientes con un valor igual o superior a 60 son capaces satisfacer la mayoría de sus necesidades, mientras que aquellos con un valor igual o inferior a 30 están totalmente incapacitados y necesitan tratamiento de soporte activo.

Analizando el conjunto de los pacientes se observó una relación significativa entre los valores iniciales del IK y la media de supervivencia a los 10 meses (p < 0,001), y una relación inversa entre los valores iniciales del índice y el porcentaje de mortalidad (p < 0,001).

The median age was 67 years (27-89), with median length of survival of 147 days. The RPA identified four distinct survival groups (p < .0001) with three variables: KPS, physical well-being, and physical symptom distress. The most significant split was KPS of 50%, followed by physical well-being score of 25 and physical symptom distress score of 0.6.

The Karnofsky Performance Status

The Karnofsky score is a simple and rapid method to assess patients’ performance in ADLs, which has been mostly used in medical oncology. The score has proven useful not only for following the course of the illness, but also for obtaining prognostic information. Patients with the highest (best) Karnofsky scores at the time of tumor diagnosis have the best survival and quality of life over the course of their illness.
