Aby uzyskać odpis aktu (certyfikatu) urodzenia, adopcji, śmierci, małżeństwa albo zawarcia związku partnerskiego w Anglii i Walii można się zarejestrować na stronie Centralnego Urzędu Stanu Cywilnego (the General Register Office (GRO)). Tą drogą najłatwiej i najszybciej można zamówić potrzebny nam odpis. Dodatkowo możliwe jest zamówienie odpisu pocztą, telefonicznie oraz osobiście w urzędzie.
Opłata od wydania certyfikatu/aktu wynosi £9.25.
UWAGA: Inaczej procedura wygląda w Szkocji i Irlandii Północnej.

W moim linku jest taka ciekawostka, że nie wiadomo, z jakiej/której instytucji pochodzi drugi formularz. Możliwe, że someone from UKMBD znalazł wyjaśnienie.

I do not recognise the application form - if this form is from the UK government, then I would recommend that you ask via one of the government's own websites. UKBMD is just a website run by volunteers to provide links to genealogy websites with data online. I initially thought that ROLO may have been one of the local register office codes, but this is not the case, so I am not able to assist any furthe

Registration districts were originally created for the registration of births, marriages and deaths, rather than specifically for the census. In many cases, registration districts were broadly equivalent to parishes, but not necessarily so.
