"Boleta", en este caso, se refiere a una "Report Card"; justo después de "Boleta" viene el término «interna».
La frase reza así:
"BOLETA INTERNA CORRESPONDIENTE AL MES DE JUNIO"

Without further information on what the "Calificaciones" refers to, I'd probably just use the literal translation qualifications, or possibly "ratings".

PS: Although Google translate gives "report card" as the translation of "boleta", It can mean other things as well, such as voucher or coupon, and if I'm not mistaken, it can mean receipt or invoice in at least one Latin American country.
