"Boleta", en este caso, se refiere a una "Report Card"; justo después de "Boleta" viene el término «interna».

La frase reza así:

"REPORT CARD CORRESPONDING TO THE MONTH OF JUNE" Es la opción que se me ocurre, pero «Interno/Interna» francamente no tengo idea de por qué les llaman así o si sólo será algún modo formal de escritura.

Without further information on what the "Calificaciones" refers to, I'd probably just use the literal translation qualifications, or possibly "ratings".

PS: Although Google translate gives "report card" as the translation of "boleta", It can mean other things as well, such as voucher or coupon, and if I'm not mistaken, it can mean receipt or invoice in at least one Latin American country.

On the other hand, if it is a school report card, as Phil suggests, "Calificaciones" could be grades (USA), marks (UK) or score... etc.
