This is in a birth certificate from Ecuador in a sentence at the end. The entire sentence reads: Declaró (the father) además que hace constar el nombre de la madre en virtud de la autorización conferida por ella la que se archiva, y ser el primer parto. The wording has me lost as to the real meaning of this statement. Is it just to record her name and that it is her first childbirth?

"En virtud de la autorización conferida por ella" is pretty clear.
"La que se archiva" refers to the authorization.

It is saying that the father has the mother's permission to declare that she is in fact the mother, that it is her first child, and that a written document in which she authorizes the father to speak on her behalf is on file (somewhere).

Yep, thanks, Neil.In UK usage, it's normally titles which are conferred, not permissions.

"is present continuous as 'is put on record'"???? the present continuous would be " IS BEING PUT". What is being referred to is obvious even to a blind man, but apparently not to you. An unbelievably cretinous response, even by your standards.
