I should have my head examined is the usual statement. The only way to tell you what have my "thread" examined means is to have more context. Maybe it's said by a seamstress, ie someone who sews clothes.

"I should have my thread examined"

The normal expression is, "I should have my head examined." Which means that the person is aware of the fact they are making some kind of decision that is illogical or that carries high risk. "Thread" does not make any sense in a plain context. Since it rhymes with "head" then the pun is understood. "Thread" has to do with fabric, sewing, textiles. If the person who made the statement is involved in any of these activities, then they are just making a play on words, a pun.
