informations écrites concernant le patient de manière relationnelle pour en assurer une lecture rapide et efficace
Cela peut être un énoncé concis qui attire l'attention du soignant sur le ou les
problèmes du jour que présente le patient, complétant ainsi le diagramme de soins autant que de besoin.

PRINCIPES DES TRANSMISSIONS CIBLEES
Une cible est une observation infirmière écrite dans le dossier de soins qui va mettre en évidence :
Une préoccupation de la personne soignée (ex : inquiétude)
Un comportement de la personne soignée (ex : repli sur soi)
Un changement dans l'état du patient (signes ou symptômes) (ex : omissements,
hyperthermie, diarrhée, douleur...)
Une réaction aux thérapeutiques ou aux soins (ex : douleur)
Un diagnostic infirmier (ex : altération de l'état cutané)
Un détail concernant les données du diagramme (ex : extravasation)
Une référence à un autre professionnel de santé (ex : kiné)
Un événement inhabituel ou inattendu ou significatif (ex : chute)
Une cible n'est pas :
Un diagnostic médical
Un acte de soins

Do que entendi, penso que se trata de um registo feito por um enfermeiro sobre o doente que está a seu cargo, dirigido ("ciblé"), numa passagem de turno e, não só, a outro enfermeiro ou prestador de cuidados.
