Vårdkontakt is indeed the (health) care contact or even point of contact, however in this context I would translate the phrase "Vårdkontakten avslutas nu..." with "Care is now concluded...".
Example sentence:
Care is concluded at the six week postnatal visit.
You are paid while out of work (TTD) and will receive a permanency reward (PPD) when your medical care is concluded and a physician has given us a report.
