Queste infezioni sono associate a tassi di mortalità che arrivano anche al 50%. Sembra che la gravità e la frequenza delle infezioni invasive da streptococco siano aumentate negli ultimi decenni malgrado si sia assistito ad un declino generale dei casi grazie all’impiego degli antibiotici.
L'aumento di malattie invasive da un lato e la comparsa di antibiotico resistenza dall’altro, è indice di un processo evolutivo rapido e significativo dovuto ad eventi genetici, tra i quali, il trasferimento genetico orizzontale che ha portato alla comparsa di nuovi ceppi con nuove caratteristiche fenotipiche importanti nell’interazione ospite-parassita.
In questo contesto, i batteriofagi temperati giocano un ruolo certamente importante.
In S. pyogenes, i geni che conferiscono la resistenza ai macrolidi mediata da sistemi di efflusso e la resistenza alle tetracicline mediata da alcuni fattori di protezione ribosomale sono contenuti in profagi funzionali inseriti nel genoma del batterio. Ad oggi, tuttavia, non si ha una dimostrazione definitiva di trasferimento genetico orizzontale di determinanti di resistenza agli antibiotici mediato da batteriofagi.
Translation - English Severe infections due to Streptococcus pyogenes are on the increase in several countries throughout the world, especially in pediatric patients. Such infections are associated with mortality rates of up to 50%. The severity and frequency of streptococcal invasive infections appear to have risen over the last few decades, despite a general decline in the number of cases thanks to antibiotic use.
The increase in the number of invasive infections on the one hand, and the emergence of antibiotic resistance on the other reflect a rapid and significant evolutionary process due to genetic events, such as horizontal gene transfer, which has led to the appearance of new strains with new phenotypic characteristics that have significant effects on host-parasite interactions.
In such context, temperate bacteriophages certainly play an important role.
In S. pyogenes, the genes conferring efflux-mediated macrolide resistance and tetracycline resistance mediated by some ribosomal protection factors are carried by functional prophages inserted in the bacterial genome. Per molti anni il tessuto adiposo bianco è stato trascurato dai ricercatori per diverse ragioni. Tra queste, sicuramente ha avuto un peso importante il fatto che la sua distribuzione nel corpo dei mammiferi è diffusa, apparentemente mal localizzata, quasi a riempire gli spazi interstiziali tra i vari organi come una qualsiasi struttura connettivale. Esso infatti risulta distribuito sia a livello cutaneo (derma), sia a livello sottocutaneo, sia a livello peri-viscerale (mediastinico, retroperitoneale, intraperitoneale: omentale, mesenterico ecc.) o intra-viscerale (parotide, midollo osseo, paratiroide, pancreas). Quando fu scoperta la natura cellulare di questo tessuto connettivo l’ interpretazione funzionale fu rapida ed intuitiva: dato il suo alto contenuto di grasso (trigliceridi) esso doveva rappresentare un deposito energetico da utilizzare nei periodi di digiuno più o meno prolungato e quindi, in qualità di magazzino non utilizzato nella routine fisiologica quotidiana, non poteva avere una grande importanza.
Anche l’ aspetto istologico non appariva particolarmente interessante: cellule sferiche di 70-90 microns di diametro con un sottile citoplasma che circonda un' unica goccia di grasso (trigliceridi). Scarsi risultavano gli altri elementi citologici presenti nel tessuto: macrofagi, mastociti e fibroblasti ed anche la vascolarizzazione e la innervazione non risultavano particolarmente interessanti.

White adipose tissue is distributed at the cutaneous (dermis), subcutaneous, perivisceral (mediastinal, retroperitoneal, intraperitoneal: omental, mesenteric etc.) and intravisceral (parotid, bone marrow, parathyroid, pancreas) levels. When the cellular nature of this connective tissue was first unveiled, the functional interpretation came quickly and intuitively: given its high fat content (triglycerides), WAT had to be an energy depot for short or prolonged fasting periods; however, not being used in the physiological daily routine, it could not be very important.
Its histological aspect (Fig. 1) was similarly uninspiring, consisting of spherical cells 70-90 m in diameter with a thin rim of cytoplasm surrounding a single lipid droplet (triglycerides); scarce cytological tissue elements (macrophages, mast cells and fibroblasts); and unimpressive vascularity and innervation.
The description of mammalian brown adipose tissue (BAT) had a separate history. The term adipose is due to its high triglyceride content, whereas brown is due to its darker hue compared with white fat, a colour that is visible to the naked eye and stems from its different cellular and tissue anatomy: smaller (30-50 m in diameter) and not properly spherical (polyhedral) cells with a rounded nucleus occasionally located in central position. The distinctive feature of BAT is the multilocular organisation of cytoplasmic triglycerides into multiple separate droplets that are easily seen at the light microscope (Fig. 2). The electron microscope evidences large and cristae-rich mitochondria filling the cytoplasm (Fig. 3), another distinguishing characteristic from white fat. The histological features of BAT are also different: vessels are much more diffuse and nerves appear to infiltrate the parenchyma, forming numerous neuro-adipocytic synaptic contacts. It is the high capillary density of this tissue and the large amount of mitochondria found in brown adipocytes that confer on BAT its brown colour. Despite a similar interstitial distribution, the mammalian BAT is easier to localise precisely compared with WAT, especially in smaller animals.
The function of BAT—heat production—and its large proportion in hibernating animals were described in the 1960s. This led to circumscribe the importance of this tissue, also because it was increasingly clear that its amount was negligible in large mammals, man included.
