Source text - English Atrial natriuretic peptide (ANP) is a 28 amino acid peptide
stored and released by atrial myocytes in response to atrial
distension and stretch. In the normal heart, ventricular tissue
produces only minor amounts of ANP, but major amounts are
found in ventricles of fetuses and in patients with left ventricu-
lar (LV) dysfunction.
In the last decade, plasma concentration
of ANP has been used a marker of heart failure in paediatric
population.

Heart failure in children and infants with rheumatic mitral
regurgitation and congenital heart diseases with left to right
shunt is caused by volume overload. The increase in ANP level
is attributed to atrial stretching.
3 Increased circulating concen tration of ANP has been correlated both with elevated cardiac filling pressure and severity of ventricular systolic dysfunction, 4 but there is limited data about the ability of ANP to detect clinical decompensation in absence of systolic dysfunction as well as the prognostic value of ANP regarding the clinical functional class and LV dimensions.
Translation - Turkish Atriyal natriüretik peptit (ANP), atriyal genişleme ve gerilmeye yanıt olarak atriyal miyositler tarafından depolanan ve salınan 28 amino asitli bir peptittir. Normal kalpte, ventriküler doku sadece az miktarda ANP üretir, ancak fetüs ventriküllerinde ve sol ventrikül (SV) disfonksiyonu olan hastalarda fazla miktarlarda bulunur1. Geçtiğimiz on yılda ANP’nin plazma konsantrasyonu çocuk popülasyonunda kalp yetmezliğinin bir belirteci olarak kullanılmıştır2. Romatizmal mitral yetersizliği ve soldan sağa şant ile konjenital kalp hastalıkları olan çocuk ve bebeklerde kalp yetmezliği volümün fazla büyümesinden kaynaklanır. ANP seviyesindeki artış atriyal gerilmeye atfedilir3. Dolaşımda artmış ANP konsantrasyonu, hem artmış kardiyak dolum basıncı hem de ventriküler sistolik disfonksiyonun şiddeti ile ilişkilendirilmiştir4. Fakat, ANP'nin sistolik disfonksiyon yokluğunda klinik dekompansasyonu tespit etme yeteneği ve ANP'nin klinik fonksiyonel sınıflandırma ve SV boyutları hakkındaki prognostik değeri ile ilgili sınırlı veri vardır.
