sin que desde dicha fecha y hasta la actualidad se haya producido incidencia alguna en nuestras relaciones bancarias con dicho cliente

this is my translation:
without any incident whatsoever arising in our bank relationship with the said client since that date to the present time

What comes before is:

Don (name) CERTIFICO que (name) es cliente de (name of the bank) desde (date), sin que desde dicha fecha y hasta la actualidad...

Without there being any (negative) impact

Incidencia can mean "impact" or "effect".
