Переводите описательно или думайте-ищите в направлении damaging pruritus, skin damaging itch/scratch. "Биопсирующий зуд" и по-русски то архаический термин, почти не употребляется. А уж "biopsy itch" вообще не на те мысли наводит (наглядный пример -- восприятие термина Judith Hehir: к биопсии ведь он не имеет отношения).

Можно, наверное, и excoriating itch, но экскориации -- это такие вторичные элементы сыпи (ссадины, царапины), которые бывают при любом зуде, когда человек расчесывает зудящее место. А при зуде чешется всегда. Мне кажется в "биопсирующий" создатели термина закладывали более глубокие повреждения кожи.

Ну вы же сами так хорошо и понятно его описали: "itch that drives the patient to damagE the skin by scratching". Зачем же заменять частное (skin) на общее (tissue)? Впрочем, это всего лишь мое личное мнение. И на английский я не перевожу, и вашего контекста не вижу.

unbearable
"биопсирующий зуд" obviously makes no sense. I would use smth like "unbearable" to express an irresistible drive to scratch oneself

skin scratch-off itch
The itching will NOT stop until I have scratched off the top layer of skin, leaving an open place that is then very painful for the next few days until it starts to heal.
