It's a term that appears towards the end of partially-standardised French birth certificates (each Mairie seems to have a different version though).
The biological father may not declare himself (please excuse me, I forget the exact wording), so when and if he does, that bit goes in here.
I translated a lot of these for certified translations over an 18-month period recently.

I had a previous translation, but I forgot what it was, before I came up with this one. It's a tricky one.
I don't know if you could actually simplify it to something about parents or parentage, because that's what it amounts to, really.
