French to English Medical Medical (general)
From a document containing blood test results. This comes under information at the top, including patient number etc.:

HOSPITAL XXX
**ADDRESS**

Prescrit par: Homonymie de prescripteurs

Further on after "Prescrit par:" it simply gives the name of the doctor requesting the blood test.

I cannot think what this means!

Unable to identify requestor unambiguously

I think I saw your document. I was asked to translate it a couple of days ago but was too busy to take it. Apparently, the request slip for diagnostic testing was signed by requesting doctor's initials (or maybe the surname alone, though the initials are more likely), and there are several local doctors with the same initials or surname so the data entry operator could not identify the requestor.
